Senior Quality Improvement Facilitator

responsibilities:

The Senior Quality Improvement Facilitator is responsible for coordination and implementation of quality improvement initiatives. This includes researching data, developing project plans, monitoring quality improvement activities, individual case or record review, serving as a resource, and providing education to team members, providers, and practitioners.

What You'll Do:

  • You will facilitate health care activities, project participation, or review and implementation with physicians, providers, and team members, ensuring plans, tasks and timelines meet Telligen and contracting agency requirements.

  • You will identify, spread, and provide education on improvement methods and processes to providers and practitioners and assist in implementation.

  • You will develop and maintain activity and/or project plans. You will also ensure plans, tasks, and timelines meet contractual and Telligen requirements and guidelines. You will provide regular updates to Telligen leadership on activity and/or project status.

  • You will research and gather technical and clinical data to support activities and/or project initiatives. You will perform an analysis of the results of the data collection. Based on the outcome of the analysis, you will determine areas for improvement.

  • You will provide recommendations to clinical partners to improve care delivery.

  • You will assist in the development of project materials and intervention tools.
