Scheduled Hours: PRN, 7:30am-4:00pm
Job Description:
Delivers Palliative Care to patients and families working in collaboration with a physician and other palliative care team members. Serves in a consultant role, acting as an advocate for patient advance care planning, identifying, and communicating patient goals of care, pain, and symptom management, and identifying patients when appropriate for hospice care. Provides high quality care to patients with serious or life limiting illness of all ages, regardless of whether care is for comfort or curative.
Qualifications:
Required:
Graduate of an accredited school of nursing
Preferred:
Hospice and Palliative Care Nurse Certification
Minimum of three years hospice or palliative care experience.
Acute Care nursing experience helpful
