The Quality Improvement and Outcomes team is looking for a Service Options Manager to meet the needs of internal and external customers, stakeholders, and individuals receiving services. Support individuals, their families and/or other stakeholders in understanding the service options offered at EPI. Managing the initial requirements for starting services, analyze referrals, write intake plans, and start and end of service data in conjunction with the QIO Director.

Responsibilities:

  • Maintain an in depth understanding of service options and referral sources for people based on their eligibility and funding

  • Provide literature and virtual imaging of services available through the agency

  • Provide education and consultation related to service options and eligibility

  • Provide management and support related to housing options

  • Write Individual Service Plans for persons starting services

Required Knowledge, Skills, and Abilities

  • Proficient in word processing, database management and computer skills

  • Demonstrated ability to represent the organization in a professional manner including exemplary written documentation, clear and professional verbal communication and ability to work effectively with internal and external stakeholders

  • Knowledge of eligibility requirements for service options and funding sources. Knowledgeable of community resources, information and referral practices and problem-solving techniques.

  • Ability to provide public awareness, education and advocacy about the needs of individuals with disabilities.

  • Ability to administer functional assessment tools.

  • Ability to participate in intake meetings, events, training sessions, discharge meetings, etc. (flexible hours, including evenings, overnights and weekends throughout the region and state)

Qualifications

  • Bachelor's degree in social work, human services, or related field plus three (3) years' experience in service delivery for people with disabilities in conjunction with supervision of services OR

  • Combination of related education and experience equal to seven (7) years

  • Valid driver's license, reliable vehicle, and current auto insurance
