Through theory-based practice, the RN utilizes critical thinking to assess, plan, implement and evaluate outcome based care for a select group of patients by phone. Determines appropriate level of care access. The RN facilitates communication with physicians, patient and family, other departments and/or team members. Adheres to MercyOne performance expectations and performs all defined services and other related duties in accordance with the mission, vision and values of MercyOne.

What you will Do:

  • Receives, reviews and acts upon hospital discharge report showing patients who have not scheduled required or recommended follow up visits

  • Receives, reviews and acts on Reports for ACO and PCMH

  • Conducts pre-visit chart review of targeted patients

  • Identifies patient short term goals and develops them into an outcome based plan of care

  • Evaluates patient response and modifies the outcome based plan.

  • Educates patient and family needs

  • Maintains professionalism when assisting callers

  • Provides care appropriate to the age related needs of the patients based on principles of growth and development.

  • Adhere to all safety, infection and employee health polices and procedures

*Hours/Schedule: *   Part-Time

48 hours biweekly (.6 FTE)

*Minimum Qualifications: *    

  • Associate Degree: Graduate of accredited school of nursing

  • Bachelor’s Degree preferred

  • Minimum of 3 years nursing experience

  • Current Iowa RN license. Mandatory Reporter
