Trinity Health Clinical Pharmacist 2 - Family Med Faculty (Full-Time) in Waterloo, Iowa

Description:

An integral part of the Family Medicine Graduate Medical Education (GME) Faculty in Waterloo, Iowa. Works closely with the Family Medicine Residency Program Director and Faculty to build a curriculum for the Residents to learn more about pharmacotherapy and other pertinent pharmacy topics. Said curriculum will need to be updated regularly which includes any educational materials provided to the residents. Required to come up with ideas for educational programs and give lectures to the residents. Will be available during patient care in the clinic and attending hospital rounds to answer any questions that arise regarding medications, interactions for resident learning.

What you will do:

  • Knows, understands, incorporates, and demonstrates the Trinity Health Mission, Vision, and Values in behaviors, practices, and decisions.

  • Delivers internal consultative services related to Trinity Health and RHM’s specialty-related strategic goals to improve clinical and operational performance around pharmacy.

  • Directs and leads the pharmacy and quality activities in Family Medicine GME Education. Mentors, coaches, and leads residents by providing feedback and advising on pharmacy and quality performance. Quality includes Chronic Care Management work, reviewing data from our EHR, chart auditing, and other quality initiatives.

  • Creating and updating policies around teaching the pharmacotherapy curriculum to the residents, in consultation with the Program Director and other faculty. Updating of that curriculum as needed to meet new standards of care and new innovations in Pharmacy. Will provide guidance updates on new drugs, newly approved uses, and new findings regarding safety and efficacy of medications.

  • Demonstrates leadership, collaboration, critical thinking, and teamwork when facilitating clinical standard and operational decisions and resolving issues.

  • Utilizes data/reports to help the residents look at populations of patients by age, gender, sex, ethnicity, and race, disease processes, and other factors to determine underserved populations or populations we need to reach out to so we can improve their outcomes. Helps us achieve our annual goals set for caring for diabetics, hypertensives, and other chronic conditions.

  • Provides precepting and mentoring support to PGY1/PGY2 pharmacy residents. Works with the MercyOne Waterloo Medical Center Residency Program to provide ambulatory care opportunities and research support.

*Hours/Schedule: *   80 hours biweekly (1.0 FTE)

Minimum Qualifications:

  • Must possess a comprehensive knowledge of pharmacy (medications) through equivalent combination of education and experience.

  • Must have relevant credentials as defined below:

  • Pharm.D. degree from accredited college of pharmacy.

  • PGY-1 pharmacy residency

  • PGY-2 or Board Certification preferred.

  • Pharmacist license current in the State of Iowa, or eligible.
