Position Purpose:

Job Description Details:


Provides nursing services to patients and families at end of life that include assessment, planning, intervention and evaluation.


Education [formal]:

  • Current RN license is required

  • Completion of physical assessment course preferred

  • BSN preferred

  • Hospice and Palliative Care Certification preferred


  • Must have demonstrated nursing competence in an acute/long-term care patient care setting

  • Caring for terminally ill patients/families, Hospice or home care nursing experience is preferred

Special Skills and Competencies:

  • Demonstrates empathy and belief in the hospice philosophy

  • Demonstrates ability to work independently, make autonomous decisions, yet function within a team structure

  • Demonstrates good organizational skills

  • Demonstrates a commitment to individual development by attending meetings and in-services

  • Obtain Port Access Certification within one year of hire date
