* Day 1 benefits - This role will be splint between Hospice: Quality Management Specialist 20 hours a week and Hospice Educator 20 hours a week*

Hospice Quality Management Specialist Position Purpose (20 hours):

​Identifies specific performance improvement initiatives and works collaboratively with the local agencies/leaders in their development, implementation and evaluation of quality improvement programs. This position identifies, and collaboratively monitors the implementation of identified processes, to assure continued agency licensure, accreditation and coverage compliance requirements. Performs identified workflow review and completion to achieve standardization and organizational efficiencies. Works collaboratively with agency, assuring compliance with new regulations, standards in EMR documentation.

Hospice QMS: What You Will Do:

  • Supports in-service education and other continuing education activities related to quality, outcomes, QA/PI. Monitors publicly reported outcomes and other data, utilizing quality software and EMR reports.

  • Assures agency staff compliance with federal/state and accreditation regulations through record review, case conferencing, mentoring and survey readiness/oversight functions.

  • Collaboratively leads agency MDQI program, developing/ implementing QAPIs, action plans to attain top decile outcomes, patient satisfaction, strategic goals and regulatory compliance.

  • Conducts clinical record review, focused quality audits, analysis of OBQI/OBQM reports other reports to assure documentation accuracy, regulatory compliance, utilization of services, promotion of patient centered care and achievement of clinical outcomes, metrics, regulatory and timeliness requirements.

Hospice QMS: Minimum Qualifications:

  • Ability to consistently demonstrate commitment to the mission and Organizational Code of Ethics and adhere to the Compliance Program.

  • Registered Nurse with current license from respective State Board.

  • Bachelor’s Degree required.

  • Four years' experience in homecare. Minimum 2 years in a supervisory, oversight role preferred.

  • Certification: OASIS Certification required within 15 months from date of hire. Must maintain recertification with no lapse in certification period

Hospice Educator Position Purpose (20 hours):

​The hospice/home care educator is responsible for the assessment, development, implementation and ongoing evaluation of education for home care and/or hospice with an emphasis on ensuring compliance with local, state and federal regulations. Works in collaboration with the hospice and/or home care managers to ensure orientation, competency development and integration of new colleagues as well as the ongoing development of current staff. Additional responsibilities include serving as the informatics liaison and providing education regarding competence with the multiple types of technologies (computer, electronic health record (HCHB), learning management system, etc.) that are required for each role. The educator also functions as the liaison with schools, colleges and universities to coordinate student learning activities.

Hospice: Minimum Qualifications:

  • Graduate of an approved Nursing education program, bachelor’s in nursing is preferred.

  • Ten years of nursing practice including five in Hospice is preferred. Related experience in end-of-life care, home health or oncology is a plus.

  • Certification in Hospice and Palliative Care is required within one year of employment.

  • CPR Instructor Certification preferred.

  • Must be proficient in the use of Microsoft applications for creation of instructional documents, course curriculums, orientation materials and related visual aides

  • Must possess accurate and clear oral & written communication skills

  • Ability to consistently demonstrate commitment to the mission and Organizational Code of Ethics and adhere to the Compliance Program.
