UnityPoint Health Supervisor of Cardiovascular Outcomes in Cedar Rapids, Iowa

Provides leadership support as part of the Heart Center management team, supporting the Heart Center Manager in their responsibility for coordinating activities required to meet the department’s internal and external data related needs as well as supporting the development of care provided by all Heart Center departments consistent with the established strategic plan, goals and objectives for Patient Care Services.

This individual provides support to department leaders regarding development, implementation and evaluation of care and management of Heart and Vascular Institute (HVI) process improvement efforts. The position facilitates department process improvement by supervising, educating and training clinical data abstractor staff on the use of the registry data including but not limited to the Society of Thoracic Surgeons (STS), and the American College of Cardiology National Cardiovascular Data Registries. This individual collaborates with various UnityPoint Health team members participating in the accreditation / certification review process to ensure that all standards are met and maintained within the organization for the Society of Cardiovascular Patient Care (SCPC), as well as any other accreditations pursued within the HVI. In addition, this individual will support and facilitate the research study activities of the HVI.

As a Supervisor this individual carries out the day-to-day operations plan of the Cardiovascular Outcomes department, under the direction of the Heart Center Manager and/or Cardiovascular Director. The Cardiovascular Outcomes Supervisor serves as a clinical resource to other healthcare team members across all Heart Center departments and may assist with patient care, program development and staff education based on department needs.

Cardiac Registry Oversight

  • Coordination of HVI data abstraction and registry maintenance:

oCoordinates concurrent and retrospective analyses of patient records for specific cardiovascular regulatory submission requirements.

oMaintains data dictionary of defined cardiac outcome measurements, terms, and calculations.

oEnsures patient data is entered into the cardiovascular databases, audits data for completeness and accuracy and generates data files for review and submission.

oReports progress of data abstraction efforts routinely to department manager and service line director as needed and coordinates abstractor’s efforts to ensure data deadlines are met.

oEstablishes and produces quarterly, monthly, and other scheduled reports in a timely manner and will share data with physician leaders and applicable committees as needed.

  • Supports excellence in Clinic Practice within the HVI:

oCoordinates the bimonthly door to dilation multidisciplinary team review of emergent Cath Lab cases.

oParticipates in Mission Lifeline efforts for STEMI care improvement.

oShares MI specific data with primary local EMS agencies for process improvement in the care of the chest pain patient population.

oProvides STEMI Cath Lab alert case-specific feedback to EMS providers includes specific times for the various components of the patient’s care, specifics of treatment in the Cath Lab, before and after pictures, and areas of success and / or opportunities for improvement.

oCommunicates with referral facilities to provide case review of patients transferred for care

oCoordinates regular visits with referral hospitals.

oAssists with regional outreach efforts for Heart and Vascular Services.

  • Supports quality improvement processes for the HVI:

oCoordinates the evaluation of programs and projects for effectiveness.

oProvides consultation for process improvement data collection tools.

oInvestigates best practices to decrease unnecessary variation in care and improve patient outcomes.

oFacilitates queries and reports for process and quality improvement efforts.

oEducates physicians, staff and leaders in the use of registry data and the evaluation of evidence-based medicine.

oBuilds collaborative partnerships to promote and support research and outcome studies of patient care.

oEvaluates clinical practices in light of national standards and benchmarks.

oProvides leadership and coordination to multidisciplinary teams focusing on designated registry populations.

  • Evaluates daily staffing needs. Collaborates with the leadership team to facilitate adjustments that meet the department needs.

Clinical Excellence/Patient Care

  • Assumes primary responsibility for the plan of care for a patient or group of patients, per unit standards and nursing care required for the specified patient population.

  • Effectively delegates tasks and responsibilities to other health care team members and assumes responsibility for follow-up and collaboration.

  • Performs admission assessment process per specific department standards to include patient history, physical, and psycho-social assessment. Utilizes physical assessment data, diagnostic data (lab, radiology, diagnostic testing), pharmacological information and patient/family perceptions in formulating nursing diagnosis related to focus problem areas and identifying goals of care.

  • Assumes responsibility for developing a plan of care that is mutually established with the patient and family. Each patient is treated as an individual, with their identified goals and expectations reviewed each shift to verify priorities.

  • Implements the plan of care according to current standards of nursing practice, evidence-based practice, and hospital protocol/procedures.

  • Integrates multidisciplinary approach to maintain comfort, support human functions and responses, and maintain an environment conductive to safety and well-being. Implements measures to promote a safe environment.

  • Partners with healthcare team to achieve the patient care outcomes and goals of care.

  • Administers medications and nursing treatments within scope of practice.

  • Incorporates St. Luke’s Comfort/C.A.R.E. Professional Practice Model in their daily nursing care.

QUALIFICATIONS:

Experience:

Three years of RN experience in a clinical setting.

Recognized knowledge and expertise in caring for cardiac specific patient populations.

Possess current RN licensure in the State of Iowa or compact state.

Maintain current BLS certification.

Maintain current ACLS / Core certification.

Valid driver’s license when driving any vehicle for work-related reasons.
