Position Purpose:

Under limited supervision, provides appropriate discharge planning and other social work services to patients and their families which addresses the patient’s psychosocial needs and concerns, as related to their illness disability, and/or hospitalization. Enhances patient/family’s problem-solving capabilities by linking them with available resources and services. With continuity of care as a goal, participates in the interdisciplinary team process on assigned nursing units and communicates with other health care professionals as an advocate for the patient/family.

What you will Do:

  • Conducts timely interviews with patients and/or families to obtain information for the purpose of needs assessment.

  • Serves as a key member on interdisciplinary teams, assisting medical and nursing staff in understanding the significance of social and emotional factors, which may affect the patient=s coping and eventual recovery. Contributing an expanded knowledge base of patient/family functioning, gained in contact during unit specific services; thereby, ensuring smooth transitions between inpatient and outpatient services.

  • Assesses patient/family need for financial assistance. Advises and assists them in appropriate application procedures for the types of benefits for which they may be eligible under these programs.

  • Provides community based referrals to suitable support services to enable patient/families to cope with the ramifications and unanticipated consequences of their chronic illness.

  • Prepares documentation for patient’s medical record, in the form of progress notes, which appropriately reflects contacts, referrals, discharge plan, or any other business conducted with or for the patient/family.

  • Functions as an advocate for special needs of patients/families in relationship to issues of culture, ethnicity, and language diversity.

  • Serves as an advocate to preserve patient’s rights.

  • Provides back-up coverage and support for social work services and discharge planning in inpatient and outpatient nursing units as identified and assigned.

  • Meets regularly with the program director and other members of the department for the purpose of conducting regular business, case review, staff and program development, discussion of departmental policy and procedure matters, and participation in the Performance Improvement activities.

Minimum Qualifications:

  • Requires a Bachelors of Social Work (B.S.W.) or Bachelors Degree (B.A.) in a related human service field from an accredited school.

  • Requires current licensure at the Licensed Bachelor Social Worker (L.B.S.W.) level from the state of Iowa or qualify to obtain such designation through reciprocity or testing, within a reasonable period of time.

  • Experience in a health care setting, with an interdisciplinary approach is desirable.

  • Has familiarity with Federal, State, Regional, and Community Resources. An established network is desirable.

  • Possesses effective verbal and written communication skills.

  • Has an understanding and commitment to the philosophy, mission, and vision of Mercy.
