Great River Health Systems Social Services Specialist | 40 hours per week | Park Place Elder Living in Mt. Pleasant, Iowa

Scheduled Hours: Day shift, M-F

Job Description:

Coordinate Care Plan meetings and assist with the Care Plan process. Schedules resident physician appointments, coordinating transportation needs. Coordinate admissions and discharges. Assists other departments for resident and/or caregiver social service’s needs. Serves as a resource counselor. This position is intended to assist residents and their caregivers with transition into the long-term care environment and to accomplish required tasks, responsibilities and skill required of individuals assigned to this position. Assist with in-services. Asist with Medicaid applications and have general knowledge of Medicaid process. Assist with and file long term care insurance.

Qualifications:

Education

  • High school diploma or GED.

  • Preference for bachelor’s in social work degree, or related equivalent experience.

  • Maintains current and active status on all required training and education as outlined by corporate policy.

Licensure

  • Emergency First Response required. Upon hire, must be completed within 60 days.

  • Dependent Adult Abuse required. Upon hire, must be completed within 180 days.

Experience

Minimum Required Qualifications

  • Experience working with elderly population preferred.

  • Prefer work experience or training with Microsoft Suite products (Word, Excel, PowerPoint, Publisher, Outlook).
